Kjøstvet har skrevet forord til alle kapitlene samt et omfattende kommentarverk som korrigerer Hitlers opplysninger og påstander. Hitler skrev boken i to bind. «Et oppgjør» ble første gang utgitt i Tyskland i 1925, og andre bind, «Den nasjonalsosialistiske bevegelse» utkom i 1926. I mai 1930 utkom Mein Kampf for første gang i en ettbindsutgave, og frem til krigens slutt ble det solgt 12,5 millioner eksemplarer. Den nye, norske utgaven er komplett med begge bind.
