In recent years, cytology has played an increasingly important role in the diagnosis of disease, particularly those of neoplastic origin. It is not unusual for cytologic specimens to be the only diagnostic samples available from patients with cancer. Many ancillary tests traditionally performed on histologic material are now being performed on cytologic specimens. One such technique, immunocytochemistry (ICC), has already proved to be important in diagnostic tumor pathology. This atlas examines the value of ICC as well as its limitations in contemporary cytology. Methods have been refined and high-quality reagents are now available. As a result, technical problems no longer present a major concern in this field. This volume will briefly address the technical aspects of ICC by providing practical advice for the users and analytical aspects of ICC, including correct selection of markers for specific differential diagnoses and incorporation of results in final cytologic interpretation; illustrate the utilization of ICC in the resolution of some common diagnostic problems in fine needle aspiration and body cavity fluid cytology. ICC of undifferentiated malignant neoplasms, carcinomas of unknown origin, and most importantly its utilization in specific differential diagnoses that are based on cytomorpholoy and patient’s clinical history is also discussed. The last three decades have witnessed enormous progress in the care of patients with congenital heart disease. Treatment of congenital heart disease is highly dependent on technology and much of the progress we have witnessed is attrib­ utable to technological advances we take almost for granted today. It would be difficult to overestimate the impact of these advances; noteworthy examples include the development of 2-D Doppler echocardiography resulting in increased diagnostic accuracy, improvements in preoperative management including the use of prostaglandins for maintaining ductal patency, better intraoperative support such as the development of cardiopulmonary bypass circuits specifically designed for neonates and infants and improvements in postoperative care too numerous to delineate. As the spectrum of congenital heart disease we can treat successfully has broadened and the results have improved much of the focus has shifted, properly, to long-term issues such as neurodevelopmental outcome and quality of life. Despite the current focus on long-term outcomes we must not forget that surgery is central to our treatment strategy. The word technology is derived from the Greek word "techne" meaning craft and before any late out­ comes can be measured the "craft" of surgery must be performed with excellence. Dr. Litwin's career has spanned these last three decades and he has witnessed and participated in the evolution of congenital heart surgery. While reflux disease, achalasia, esophageal spasm, gastroparesis and IBS include some of the most common disorders in all disease categories, the understanding of their pathophysiology has remained elusive. The field of clinical gastrointestinal motility has for decades relied on the measurement of intestinal movements for diagnosis and management of these difficult and enigmatic disorders of gut function. Although computers have increased the speed with which we can measure the movements of the gut, the devices to measure this movement have not changed in over 20 years. In the last 2 years, a new technologic breakthrough has taken place in the measurement of intestinal movement. The technology is called high resolution manometry. Rather than the old 4 and 8 channels systems of measuring pressure, high resolution employs 36 closely spaced solid state pressure transducers. By using this technology, the resolution of gut motor activity is incredible. This allows for better ways of viewing motility using color as pressure. This technology makes for beautiful images of gut motility that we have never seen before. We have made diagnoses that would never have been appreciated with the old technology. High resolution manometry is taking over conventional manometry worldwide and represents a dramatic leap in a long time stagnant area.
