Oral health is known to be associated with general health. Much oral significance is closely related to systemic diseases. There are several categories of systemic patients for whom specific needs of dental care and counselling are needed. In some instances, so as to reach the optimal total health care, a combination of medical and dental care is necessary. Furthermore, poor oral hygiene itself is a risk factor for systemic diseases such as periodontal status which can increase the severity of heart disease and diabetes. Oral hygiene assessments are important so as to eliminate predisposing factors for oral complications such as nasopharynx cancer, immune-compromised disease, diabetes and valvular heart diseases in patients. Since early assessment can reduce both the incidence and severity of oral complications, some early appropriate interventions can be engaged according to the individual's treatment needs. It is necessary to carry out consistency in the treatment by including periodically follow up and appropriate counselling to the patients, so they may be able to take care of themselves up to the desired outcome. This book focuses on the oral health care for specific groups of the systemic patients i.e. diabetes, head and neck cancers, cardiovascular disease, and AIDS.

It is generally recognized that the knowledge and research base that underpins dentistry lies in the biological and physical sciences. In this context, the major advances in these sciences over the past two decades have come through the application of molecular bi- ogy and nanotechnology. These advances are currently impacting on the diagnosis and treatment of a wide range of human diseases and it is essential that dental research, education, and practice keep pace with this rapidly advancing ?eld. As pointed out by Ford et al. (1): The de?nition of disease is also changing. Previously, disease was understood to be the presence of symptoms or of a particular phenotype. With increasing knowledge of the genetic basis of many diseases, this de?nition is changing to become the presence of a genotype conferring a pre-disposition to clinical symptoms or phenotype (Ford et al. (1)). This changing de?nition of disease means that today’s undergraduate or graduate student in dentistry (and its related ?elds) must be in a position not only to acquire new knowledge in the future but also to be able to evaluate the information and apply it in a clinically relevant setting. This naturally positions oral biology as an integral part of any dentally related professional’s repertoire of knowledge. There are as many topics in oral biology as there are the number of sites and micro- vironments within the oral cavity.

For the generation that reached sexual maturity in the 1960s, the “pill” became synonymous with sexual freedom and started a sexual revolution. For women it meant freedom from the fear of pregnancy, and for men enhanced sexual opportunity. The new era of the pill has nothing to do with fertility, but everything to do with sex. The first orally effective prescr- tion drug for treating erectile dysfunction (ED) was marketed in 1998. ® Sildenafil (Viagra ) has rejuvenated the aging male veterans of the sexual revolution, forever changed the science of sexual medicine, and tra- formed society’s perspective on aging and sex. This class of drugs, known as oral phosphodiesterase inhibitors (PDE-type 5), is highly effective in the treatment of ED. Since its introduction there has been a much greater awa- ness of ED, its comorbidities, and its effects on the quality of life. In 1997, while preparing to address the Endocrine Society on the occasion of the 92nd American Urological Association meeting, I first looked at the p- clinical studies of sildenafil. I thought “this will change everything” and it clearly has—changing practice patterns in sexual medicine, and the at- tudes of patients, potential patients, and their partners. Two new PDE-type ® ® 5 inhibitors, tadalafil (Cialis ) and vardenafil (Levitra ), were first approved by the European Committee for Proprietary Medicinal Products and subsequently by the Food and Drug Administration in 2003 and 2004.

It had long been apparent that the views of mountain people were different from their off-the-mountain governments. Their differences were not often recorded accurately if recorded at all. The time seemed right and proper to make some corrections. A book written by someone who had lived the life was a somewhat remote possibility. That possibility became more real when Leonard was finally married in his sixties. Wife Martha "Marny" became a real help in achieving that end. They now live out of the mountains in Archer Lodge, NC. They moved there when NCDOT took their Deep Gap, NC home for highway improvements. This book is primarily for family and friends but delves into unique ways of living and thinking that should not be lost.
