My Red Shirt and Me The red shirt incident begins with a rather ordinary red shirt. Not a brightly colored red shirt, not a dramatic cherry or firehouse red, more like a faded burgundy. But, for several days, my very iden­ tity was bound up in its redness. It was me, and I wore it with the pride a matador takes in his splendid cape, a hero in his medals of bravery, or a nun in her religious habit. I'll never forget the bound­ less joy I felt wearing that simple, pullover, short-sleeved red shirt in the hospital--or the rush of relief that I experienced when, at last, I decided to surrender it. However, we are getting ahead of our story, which starts a short time earlier with a most unfortunate accident. A light flurry of wet snow had begun to fall as the university limousine turned the corner on its way from the Bronx campus of New York University to the downtown campus. Although eight of us were packed into the car and had resigned ourselves to the usual boring faculty meeting awaiting us, somehow a spontaneous air of joviality was created.

Three years post-9/11, Mike, a staff sergeant in the US Army, has command of a squad of ten soldiers stationed in Kabul, Afghanistan. The squad's orders are to protect the Kabul markets as the city comes back to life after years of war. The area has thankfully stabilized, forcing terrorists to hide in the surrounding mountain range. Mike, anxious for any intel on terrorist whereabouts, presses his loyal informant Hakeem. Nervously, Hakeem informs Mike of a time-honored tradition favored by warlords and corrupt businessmen: the buying of "bacha bazi boys." This work, Essential Endocrinology: A Primer for Nonspecialists, is written with dual purposes in mind: first, to provide a framework of basic endocrinology and diabetology to the medical student, and second, to provide a quick, con­ cise, and handy "guide" to the junior residents in their early years of training who wish to obtain a working knowledge about endocrine disorders that affect their patients. One of the outstanding advantages of being a teacher of en­ docrinology to students and junior residents is that it bestows a perspective from a unique vantage point. Books written for the junior members of our profession have suffered from extremes of caliber, ranging from excellence beyond their comprehension to insufferable mediocrity. Textbooks in en­ docrinology that are simple enough to cover the principles of that speciality and yet comprehensive enough without treading into controversial quicksand are few and far between. This book is aimed at filling that gap and is written with no other criterion than simplifying a complex subject matter. From this touchstone, the work has never really departed. A decade of experience as a teacher and physician in the field of endo­ crinologyhas impressed on me that the process of "simplification" rests on four basic principles: an understanding of endocrine concepts, the application of these concepts to the understanding of diseases, the transference of knowl­ edge to clinical situations, and the integration of the patient with the labo­ ratory, the ultimate testing ground where clinical diagnoses stand or fall.
