Chemistry is a subject experiencing very rapid growth. New areas of researeh are opened and new methods are developed with a speed whieh must seem impressive. The pieture has, however, another side represented by the great number of unsolved or poorly solved problems left behind when a branch of chemistry is no longer fashionable. When three years ago I beg;m to plan this book I felt that the molecular adduets of oxo­ compounos constituted just sueh a field of research, and that they might acquire greater interest if approached from the standpoint of modern chemistry. The developments of the last three years have been such, however, that it can no longer be said that interest is lacking. From that point of vic~w a book on this subject may now seem unnecessary. On the other hand individual contributions to this field have been very scattered, and the essential problems have not always been recognized. It is hoped therefore tlat this book will help to coordinate work on these compounds and that future investigations will derive some benefit as a result. For this purpose a systematie treatment is needed and the first and larger part of the book is an attempt to present a fairly complete review of past experimental work. The treatment is not detailed; it is aimed instead at giving the reader a chance to find all the pertinent references.

These notes are an extended version of lectures given in the Symposium on Mathematics and Development arranged by the School of Mathematical Sciences of the University of Khartoum, Sudan, in 1982. The purpose of the notes is to discuss some models for the transmission of tropical infections. This area of mathematical epidemiology has previously received only minor attention by mathematicians, but is now growing in importance. The term "hybrid model" is used to denote a model with both stochastic and deterministic ingredients. We describe how a hybrid model approach can be used to formulate and study both some classical models for malaria and schistosomiasis and some extensions of these models. The formulation of the models requires some familiarity with Markov chains in continuous time and discrete state space. The analysis of the models uses concepts and methods in the qualitative theory of ordinary differential equations. The presentation is aimed at the senior undergraduate or beginning graduate level.

Within the field of general medicine, the last two decades have seen the curative procedures increasingly supplemented by func­ tions that can be summed up under the heading of "preventive health care." There is a close link between this development and the changing morbidity pattern. In highly developed countries the morbidity rate attributable to infectious diseases has been much reduced, while the various types of arteriosclerosis, some tumorous conditions, and so-called functional syndromes have attained massive incidence rates. Systematic epidemiologic studies have yielded the concept of risk factors, i. e., certain exogenous and endogenous factors whose elimination, ideally, would make it possible to avoid the manifestation of an illness or to ensure it did not become mani­ fest until later in life or in a milder form. The socioeconomic and sociomedical significance of these aspects is considerable, as can be seen from all the statistics on the reasons for incapacity and early retirement and on the relative contributions of different ill­ nesses to total mortality. Effective avoidance of illnesses by primary prophylaxis (elimi­ nation of the causes of disease) or early detection of the symp­ toms of disease (secondary prophylaxis) leading to early treat­ ment would be a particularly significant step forward, since in the case of most of the conditions mentioned above only pallia­ tive forms of treatment are available, and none that have any cu­ rative effect.

Im Verlaufe der arteriellen Hypertonie sind funktionelle und strukturelle Veränderungen des linken Ventrikels und der Koronargefäße nachweisbar, die unabhängig voneinander zu einer gestörten Pumpfunktion des Herzens führen und deren prognostische Bedeutung und klinische Wertigkeit erst in den letzten Jahren erkannt wurden und hier ausführlich erörtert werden. Dies gilt vor allen Dingen für die sich schon frühzeitig und unabhängig von der Höhe des Blutdrucks entwickelnde linksventrikuläre Hypertrophie (LVH), die das kardiovaskuläre Risiko der Hochdruckkranken für einen Myokardinfarkt, das Auftreten höhergradiger Herzrhythmusstörungen und des plötzlichen Herztodes signifikant erhöht und die Prognose nach einem Nicht-Q-Infarkt signifikant verschlechtert. Die frühzeitige echokardiographische Diagnostik des Hochdruckherzens ist deshalb von großer praktischer Bedeutung und wird in diesem Buch praxisnah und in Abgrenzung gegen andere Hypertrophieursachen (z.B. Sportlerherz) unter Hinweis auf häufige Fehlerquellen anhand von Originalregistrierungen besprochen. In Langzeituntersuchungen wird erstmals gezeigt, daß eine LVH bei richtiger Auswahl des Antihypertensivums in einem hohen Prozentsatz nahezu komplett zurückgeführt werden kann und hierdurch die Pumpfunktion langfristig gebessert wird. In diesem Zusammenhang werden die der Entwicklung und Rückbildung der LVH zugrundeliegenden Mechanismen ausführlich besprochen. Auf die besondere Bedeutung der bei Hochdruckkranken gestörten O2-Bilanz des Herzens mit eingeschränktem O2-Angebot (Mikroangiopathie bzw. KHK) bei gleichzeitig erhöhtem O2-Verbrauch (systolischer Belastungsblutdruck, LVH) wird auch unter dem Aspekt der Therapie hingewiesen.

The book Free Will and Evolution defends the old notion of free will, according to which such a will is incompatible with determinism and not identical with mere indeterminism. The defense is made from an entirely secular evolutionary perspective. The theory of evolution - properly considered - is argued to be fully compatible with a belief in in a little bit of free will. Moreover, it is claimed that a complete denial normally contains a kind of contradiction. Not a logical contradiction, but a so-called performative contradiction. The denial argued for contradicts the very existence of argumentative discourses.
