This book is an introduction to the diagnosis and treatment of mental illness for the patient. The purpose of this book is to give the patient the tools to take full advantage of psychiatric diagnosis and treatment. The most important fact that the patient needs to understand is that the patient is in charge of the treatment. What the patient does has more to do with the success of the treatment than what the doctor does or what the medication does. The patient should know more about his treatment and his medications than the doctor. The doctor knows more about medicine and the practice of medicine than the patient. However, the patient need only study his mental illness and his medications while the doctor has to study all medications and all mental illnesses as well as the medical illnesses that patients bring into the doctor's office. I share my fifty years of experience practicing general medicine, emergency room medicine, and psychiatry with the client during the first office visit. This is not an urgent setting like a hospital where the client is suicidal, homicidal, or gravely disabled and the risk of not treating the client exceeds the risk of the medications. The client is encouraged to exhaust the alternatives to medications before accepting the hundreds of medication side effects that come with any medication. The client is introduced to meditation and cognitive behavior therapy during the first visit with education as to mental illness and medications is started.

The basic science of the diagnosis and treatment of schizophrenia is described from the point of view of this practitioner at the point of service with the client. I have been at the point of service since 1970 as a medical student, since 1972 as an intern, since 1973 as a licensed medical doctor, and since 1977 as a board-certified psychiatrist. The practice of psychiatry changes from place to place, and from time to time. Administrators and chain of command are at arm's length, or more than arm's length, with a different point of view. Pharmaceutical companies conduct themselves on a different platform with different rules of engagement.
