I share my fifty years of experience practicing general medicine, emergency room medicine, and psychiatry with the client during the first office visit. This is not an urgent setting like a hospital where the client is suicidal, homicidal, or gravely disabled and the risk of not treating the client exceeds the risk of the medications. The client is encouraged to exhaust the alternatives to medications before accepting the hundreds of medication side effects that come with any medication. The client is introduced to meditation and cognitive behavior therapy during the first visit with education as to mental illness and medications is started.

I have been attempting to provide quality patient care while complying with policies, procedures, and practices as a medical student, general practitioner, emergency room physician, and psychiatrist since 1970 This book distills my lifetime experience as, a child, adolescent, and adult. Context: my first memories are living on a subsistence farm with my grandmother, uncle, and two youngest aunts in the Upper Peninsula of Michigan in 1952 and 1953. After that I lived in the Shady Lane Trailer Park by 9 Mile Road and Ryan in Warren, Michigan from 1955 to 1958. My best friends were Southern Baptists from West Virginia and Alabama. I then lived in East Detroit, Michigan from 1958 to 1972 while attending Michigan State University, Wayne State University, and Wayne State University School of Medicine. I completed psychiatric residency training at Lafayette Clinic and Northville State Hospital from 1972 to 1975. I attended Wayne State University Law School in 1979 to 1983 in the night school part time program. I was Board Certified in Psychiatry in 1977 and licensed to practice law in Michigan in 1983. I have practiced general medicine, emergency room medicine, psychiatry in Michigan, Indiana, Kentucky, California, and Texas without interruption since 1972. I have worked in Regional Medical Centers, State Psychiatric Hospitals, Forensic Psychiatric Hospitals, private practice, multispecialty clinics, and prisons. I have been an expert witness in psychiatry in probate court, state and federal courts, in civil and criminal trials. Politics and money have involved a variety of stakeholders other than the patient who needs treatments and cures. Integrity is sometthing that is hard to preserve in the face of corporate politics and the pressure to build and maintain revenue streams. The electronic medical record serves billing and issues other than patient care. The electronic medical record and billing issues cause errors and burnout among health care providers.
