More than a century ago, Cripps successfully employed the direct and complete division of the anal sphincters as a means of approaching the lower rectum and anal canal, and reported on a series of 36 patients who had been treated in that fashion. Cripps was pleasantly surprised to find good fecal continence in over two-thirds of the patients during later follow-ups, despite the fact that the divided sphincters had not been repaired. The transsphincteric procedure was largely forgotten in subsequent years, however, and only the parasacral proctotomy of Kraske, which spared the anal sphincters, can be said to have gained an established place in the" surgical armamentarium. " It remained for York Mason to redirect the attention of the surgical community to the great potential of the trans­ sphincteric approach and the excellent continence that can be achieved through adequate repair of the divided sphincters. Having recognized the outstanding practical value of this procedure, we felt it necessary to define more precisely the anatomical prerequisites that would ensure minimum operative bleeding, and to bring the procedure more in line with current knowledge of normal continence and defecation. Dr. A. Huber, in consultation with the director of the Institute for Clinical Anatomy of our surgical department, Prof. A. von Hochstetter, did many months of dissection work on fresh anatomic preparations in an effort to explore and refine the various aspects of the trans sphincteric ap­ proach.

Der Zugang zum tiefen Rektum und zum Analkanal durch direkte Spaltung des gesamten Sphinkterapparates wurde vor uber 100 Jahren durch Cripps erfolgreich angewendet und von ihm eine entsprechende Serie von 36 Patienten veroeffent- licht. Cripps war dabei angenehm uberrascht, trotz fehlender 2 Rekonstruktion bei uber j der Patienten anlasslich von Spat- 3 kontrollen gute Kontinenz zu finden. Die Moeglichkeit geriet aber in der Zwischenzeit weitgehend in Vergessenheit, und im "chirurgischen Armamentarium" hat sich eigentlich nur die parasakrale Rektotornie nach Kraske gehalten, welche den Sphinkterapparat moeglichst unbehelligt lasst. Es ist das Verdienst von Y ork Mason, die mannigfachen Moeglichkeiten des sphinkterspaltenden Zugangs erneut und uberzeugend dargelegt und auf die ausgezeichnete Kontinenz- funktion bei adaquater Rekonstruktion hingewiesen zu ha- ben. Nachdem wir uns von der Nutzlichkeit des sehr pragma- tisch durchgefuhrten Eingriffs uberzeugt hatten, schien es uns unbedingt erforderlich, die anatomischen Voraussetzungen eines moeglichst blutsparenden Vorgehens zu prazisieren und dieses Vorgehen in Beziehung zu setzen zu den heutigen Kenntnissen der normalen Kontinenz respektive Defaka- tionsfunktion. Herr Huber hat sich - beraten durch den Vorsteher des Instituts fur klinische Anatomie im hiesigen chirurgischen De- partement, Herrn Prof. A. von Rochstetter- anhand frischer Dissektionspraparate uber ein Jahr mit den Fragen dieses Zugangs befasst. An den klinischen Fallen dieser Zeit konnten dann seine Erkenntnisse immer wieder in ihrer Bedeutung fur die chirurgische Praxis uberpruft werden. Die entspre- chenden gemeinsamen Erfahrungen sind in der kleinen Mo- nographie niedergelegt.

Milk quality and safety is negatively impacted by unjudicial antibiotics used in veterinary medicine because of milking animals are generally treated for infectious diseases with antimicrobial agents like tetracyclines, sulfonamides, aminoglycosides, fluoroquinolones etc. Antibiotic drug residues could have a twofold result on animal health, which may be mutagens, phototoxicity, carcinogenic, teratogenic effects, drug allergy reactions, alterations in the balance of useful intestinal microbiota, reduction in reproductive performance, selection and development of resistant bacteria, acute toxicity or poisoning in human. According to the WHO, 62 % of generally told human pathogens are zoonotic. The resistant microorganism from animals is also transferred to human via organic phenomenon. Additional acquired resistant genes might also be transferred from animal pathogens or commensals to a human pathogen.
