Plum and Posner's Diagnosis and Treatment of Stupor and Coma, 5th edition, is a major update of the classic work on diagnosing the cause of coma, with the addition of completely new sections on treatment of comatose patients, by Dr. Jan Claassen, the Director of the Neuro-ICU at Columbia New York Presbyterian Hospital. The first chapter of the book provides an up-to-date review on the brain mechanisms that maintain a conscious state in humans, and how lesions that damage these mechanisms cause loss of consciousness or coma. The second chapter reviews the neurological examination of the comatose patient, which provides the basis for determining whether the patient is suffering from a structural brain injury causing the coma, or from a metabolic disorder of consciousness. The third and fourth chapters review the pathophysiology of structural lesions causing coma, and the specific disease states that result in coma. Chapter five is a comprehensive treatment of the many causes of metabolic coma. Chapter 6 review psychiatric causes of unresponsiveness and how to identify and treat them. Chapters 7 and 8 review the overall emergency treatment of comatose patients, followed by the treatment of specific causes of coma. Chapter 9 examines the long term outcomes of coma, including the minimally conscious state and the persistent vegetative state, and how they can be distinguished, and their implications for eventual useful recovery. Chapter 10 reviews the topic of brain death and the standards for examination of a patient that are required to make the determination of brain death. The final chapter 11 is by J.J. Fins, a medical ethicist who was invited by the other authors to write an essay on the ethics of diagnosis and treatment of patients who, by definition, have no way to approve of or communicate about their wishes. While providing detailed background for neurological and neurosurgical specialists, the practical nature of the material in this book has found its greatest use among Internists, Emergency Medicine, and Intensive Care specialists, who deal with comatose patients frequently, but who may not have had extensive neurological training.

Human monitoring as a supplement to or replacement for environmental monitoring of toxic substances in the workplace has become an increasingly important issue within the last decade, leading to Congressional hearings, governmental studies, and scientific conferences around the world. Just as the purposes for undertaking human monitoring are diverse and sometimes conflicting, so too are the concerns-- medical, legal, and ethical-- such testing has generated. The authors begin by providing precise characterizations of the types of monitoring now in use and a clear account of the legal basis for OSHA monitoring requirements. They then turn to scientific and technical concerns that have evolved around monitoring, including the frequency and timing of examinations, human variability, and the distinctions that exist between high-risk and sensitive groups. Specific legal and ethical problems of conducting monitoring tests on workers are then covered in full, including the consequences for the worker of medical removal from the workplace, the conflict between human monitoring and personal privacy, access to medical records, and the use and possible misuse of test results. This book offers a new approach by combining the disciplines of history, psychology, and religion to explain the suicidal element in both Western culture and the individual, and how to treat it. Ancient Greek society displays in its literature and the lives of its people an obsessive interest in suicide and death. Kaplan and Schwartz have explored the psychodynamic roots of this problem--in particular, the tragic confusion of the Greek heroic impulse and its commitment to unsatisfactory choices that are destructively rigid and harsh. The ancient Hebraic writings speak little of suicide and approach reality and freedom in vastly different terms: God is an involved parent, caring for his children. Therefore, heroism, in the Greek sense, is not needed nor is the individual compelled to choose between impossible alternatives. In each of the first three sections, the authors discuss the issues of suicide from a comparative framework, whether in thought or myth, then the suicide-inducing effects of the Graeco-Roman world, and finally, the suicide-preventing effects of the Hebrew world. The final section draws on this material to present a suicide prevention therapy. Historical in scope, the book offers a new psychological model linking culture to the suicidal personality and suggests an antidote, especially with regard to the treatment of the suicidal individual.

Preliminary List of the Compositae in Northeastern Brazil is the fourth title to be published in this series. Many specimens from this plant family were deposited at the Royal Botanic Gardens, Kew for historical reasons and others have been added to the collections more recently, as a result of the longstanding collaboration between Brazil and Kew. Between 2000 and 2001, 4200 Northeastern Brazilian Compositae specimens were examined, resulting in a checklist of 486 species in 143 genera, with entries listed alphabetically and sorted by state, collector and number. An additional exsiccatae list enables duplicates lodged in other herbaria to be checked against the correct determination. The database that underpins this checklist is available on the internet at: www.kew.org/scihort/tropamerica/ repatriation.htm, along with images of the NE Brazilian type specimens held at Kew. Hard copies of such images are available at four Brazilian herbaria (CEPEC, HUEFS, IPA and HRB).
