William Gladstone, four-time Prime Minister of England, said of the Constitution of the United States, "...it is the most wonderful work ever struck off at a given time by the brain and purpose of man" (Wikipedia). On the day of the 2016 presidential election, the United States came close to this great document disappearing into the history books. If the democrats had won, they would have replaced it with socialism. Under this Constitution and Bill of Rights, the United States became the greatest nation in the history of the world, and people have clamored to come here for centuries to enjoy freedoms and opportunities never before offered by any nation. This book is written because liberal politicians have, for decades, been trying to replace this Constitution with socialism, a form of government tried in other nations, but found wanting. They believe a system that places everyone on a level playing field where there are no rich and no poor people is superior to life under this great document. "Everyone should share equally" is their claim, and they have been willing to eliminate the Constitution and Bill of Rights to bring this about. This book is the authors' reaction to many things done by liberal politicians, mostly democrats, to install socialism, which would change the structure of government from that of serving citizens to one where citizens would serve government. In other words, citizens would serve the wishes of politicians, and this would completely remove the freedoms we have to build lives as we desire. This would be a complete reversal of our system of government. These writings were developed primarily over the past nine to twelve years, a few before then, in response to the work of liberals in Congress to replace our constitutional republic with socialism. One thing that stands out in this is that they, liberals, would be in charge of government, so, in reality, this is a grasp for power. Many of the programs coming out of Congress during the past 60 years, and even before, were written to sound good, but which, in reality, had the effect of making people wards of the government. The welfare system they developed gave people the chance to get along in life without having to lift a finger to take care of themselves. The end result of this is that almost half our citizens are currently unemployed and useless so far as being of value in building this nation. The author believes, "My life, determined by me, will always be superior to my life determined by politicians." This is what the Constitution now makes possible. It is essential that people who understand this make certain it does not pass completely into the history books, but is strengthened so many more people will feel free to build their own dreams, not the dreams of politicians. This book is about making a great nation even greater.

Sidonius Apollinaris was an inhabitant of southern Roman Gaul in the mid fifth century AD, when it was threatened by invasions from beyond the boundaries of the Roman Empire and by competing warlords. His many poetic works include three panegyrics to emperors at the beginnings of their reigns; these are carefully translated and annotated, and provided with comment and synopses. His multiple shorter poems, in a variety of metres, are translated into good and lively English and given separate introductions and notes of various kinds, historical and literary. There is an extensive and informative introduction to the whole work. This book by Roger Green, a lifelong expert in Late Antiquity, gives a firsthand account of the political strife and manoeuvring of the times but also a vivid picture of the lives of Sidonius’s like-minded friends in an almost post-Roman episode of Rome’s existence. Sidonius was read widely in the Middle Ages, with a golden age in the twelfth and thirteenth centuries and also in the fifteenth century revival of Late Antique literature. Today his poetry will awaken new study and interest, without the archaism of many older translations and with a fresh and updated approach to many issues.

Doctors in training are exposed to pressures and distractions to which they do not always respond appropriately, and individuals and institutions can struggle to deal effectively with difficulties when they arise. This book exposes the myths surrounding medical professionalism and strips it of pretensions or exclusivity, making a complex subject accessible and easy to comprehend. It promotes best practice for dealing with unprofessional behaviours amongst doctors-in-training. Divided into two main sections, this workbook first explores topics such as what constitutes professionalism, how it might best be taught and assessed, the interactions between professionalism, ethics and legal frameworks, international trends in medical education in relation to professionalism and implications for public policy. The second section presents 29 international case studies based on real life, explores issues and makes practical recommendations. Medical educators and students will appreciate the common format with key discussion points for each case and international health and social care professionals will welcome inspiration from the candid, sincere exploration of the topic.

Dysmenorrhea and Menorrhagia

Dysmenorrhea and Menorrhagia

Dysmenorrhea is estimated to effect upwards of 70% of women during their lifetime and “heavy menstrual periods” is one of the most frequently given reasons for hysterectomy in the United States. Despite the dramatic growth of dysmenorrhea treatment with non-steroidal anti-inflammatory agents (NSADs) in the 1980s, young women continue to go undiagnosed and under treated. The diagnosis and management of heavy menstrual bleeding has even poorer general understanding because of its more complicated etiologies. With a working knowledge of the underlying pathophysiology, all clinicians should be able to establish both the diagnosis and a credible treatment plan that will result in a significant chance of clinical improvement for most patients. This book addresses the pathophysiologic underpinnings of both dysmenorrhea and menorrhagia and provides clinicians with practical approaches to establishing the diagnosis, proven tactics for successful therapeutic interventions and dealing with the challenging atypical case, and effective patient counseling. It teaches principles, including the role prostaglandins play in both conditions, as well as procedures, like surgical options and TENS, providing a balance of the practical and the esoteric. The inclusion of patient education and counseling materials additionally increases its usefulness for the busy clinician. This is an ideal guide for ob/gyn clinicians, residents, and trainees caring for patients with these common, but often misunderstood diseases.

The Wheels That Drove New York tells the fascinating story of how a public transportation system helped transform a small trading community on the southern tip of Manhattan island to a world financial capital that is home to more than 8,000,000 people. From the earliest days of horse-drawn conveyances to the wonders of one of the world's largest and most efficient subways, the story links the developing history of the City itself to the growth and development of its public transit system. Along the way, the key role of played by the inventors, builders, financiers, and managers of the system are highlighted.New York began as a fur trading outpost run by the Dutch West India Company, established after the discovery and exploration of New York Harbor and its great river by Henry Hudson. It was eventually taken over by the British, and the magnificent harbor provided for a growing center of trade. Trade spurred industry, initially those needed to support the shipping industry, later spreading to various products for export. When DeWitt Clinton built the Erie Canal, which linked New York Harbor to the Great Lakes, New York became the center of trade for all products moving into and out of the mid-west.As industry grew, New York became a magnate for immigrants seeking refuge in a new land of opportunity. The City's population continued to expand. Both water and land barriers, however, forced virtually the entire population to live south of what is now 14th Street. Densities grew dangerously, and brought both disease and conflict to the poorer quarters of the Five Towns. To expand, the City needed to conquer land and water barriers, primarily with a public transportation system.By the time of the Civil War, the City was at a breaking point. The horse-drawn public conveyances that had provided all of the public transportation services since the 1820's needed to be replaced with something more effective and efficient. First came the elevated railroads, initially powered by steam engines. With the invention of electricity and the electric traction motor, the elevated's were electrified, and a trolley system emerged. Finally, in 1904, the City opened its first subway. From there, the City's growth to northern Manhattan and to the "outer boroughs" of Brooklyn, Queens, and the Bronx exploded. The Wheels That Drove New York takes us through the present day, and discusses the many challenges that the transit system has had to face over the years. It also traces the conversion of the system from fully private operations (through the elevated railways) to the fully public system that exists today, and the problems that this transformation has created along the way.
