Written and edited by leaders in the field, this manual educates physicians on the essential components of the discipline, filtered through the experience of specialists from developing countries, with immediate applicability in the specific healthcare environment in these countries. Typically, manuals of pediatric hematology-oncology are written by specialists from high-income countries, and usually target an audience with a sub-specialist level of training, often assisted by cutting-edge diagnostic and treatment facilities. However, approximately 80% of new cases of cancer in children appear in mid- and low-income countries. Almost invariably, general practitioners or general pediatricians without special training in oncology will look after children with malignancies who enter thehealth care system in these countries. The diagnostic facilities are usually limited, as are the treatment options. The survival figures in these conditions are somewhere below 20%, while in high-income countries they are in the range of 80% for many childhood cancers. This book includes simplified therapy protocols, pain therapy and palliation, as well as ward procedures such as bone marrow aspiration/biopsies, intravenous therapy and chemotherapy drugs mixing. It provides an overview of pediatric cancer epidemiology, cancer registration and organizing support networks and features the management of cancers with associated pathology like AIDS, malnutrition, malaria and tuberculosis.

At the heart of this thesis is the young field of free electron laser science, whose experimental and theoretical basics are described here in a comprehensible manner. Extremely bright and ultra short pulses from short wavelength free-electron lasers (FELs) have recently opened the path to new fields of research. The x-ray flashes transform all matter into highly excited plasma states within femtoseconds, while their high spatial and temporal resolution allows the study of fast processes in very small structures. Even imaging of single molecules may be within reach if ultrafast radiation damage can be understood and brought under control.Atomic clusters have proven to be ideal model systems for light-matter interaction studies in all wavelength regimes, being size scalable, easy-to-produce gas phase targets with a simple structure. With FELs, "single cluster imaging and simultaneous ion spectroscopy" makes possible experiments under extremely well defined initial conditions,because the size of the cluster and the FEL intensity can be extracted from the scattering images. For the first time large xenon clusters up to micron radius were generated. Their single cluster scattering images were analyzed for cluster morphology and traces of the ultrafast plasma built-up during the femtosecond FEL pulse. The simultaneously measured single cluster ion spectra yield unprecedented insight into the ion dynamics following the interaction. The results will feed both future experimental effort and theoretical modeling.

This book questions gendered readings of violence by analyzing how this paradigm has become normalized in Italy since the feminist term ‘femminicidio’, or ‘femicide’, entered the mainstream media during the 2013 general election. It also sheds light on discourses of contestation on the part of family activists, men’s rights campaigners and divorced fathers’ groups. Two counter-discourses emerge. The first is what the author terms an ‘ideology narrative’, for which discourses built around the conceptual category of ‘gender’ normalize simplistic representations of relationships between men and women. The second is a ‘female violence discourse’, which sheds light on under-represented aggressor-victim relations and modifies dominant representations of femininity and masculinity. The author argues that integrating these two discourses into public debates helps to reappropriate the complexity and biological dimensions of (violent) relationships between men and women, often overshadowedby gender/feminist perspectives. In this way, she concludes, we can address neglected social issues that contribute to violence beyond gender. This thought-provoking book will appeal to students and scholars of sociology, critical discourse studies and gender.

This books focuses on co-design, and more specifically, on the various forms co-design might take to tackle the most pressing societal challenges, introducing public-interest services as the main application field. To do so, it presents an extensive study conducted within a particular community of residents in Milan: this is a social innovation story integrated into the discipline of service design, which simultaneously deepens the related concepts of co-design, co-production and co-management of services.Drawing upon this experience and further studies, the book presents the idea of a collaborative infrastructure and its related infrastructuring process in ten steps, in order to explore the issues of incubation and replication of services and to extensively investigate the creation of those experimental spaces in which citizen participation is fostered and innovation in the public realm is pursued.Lastly, the book develops otherlines of reflection on co-design seen, for example, as a form of cultural activism, as an instrument for building citizenship, and as a key competence for the public administration and thus as a public service itself. This book presents a systematic literature review of 156 published papers on business model innovation (BMI). The aim is to identify and integrate the different theoretical perspectives, analytical levels, and empirical contexts in order to deepen understanding of this complex phenomenon. The authors conduct an inductive thematic analysis based on an informal ontological classification that identifies 56 key themes. Within each theme, discussion focuses on thematic patterns, potential inconsistencies and debates, and future directions and opportunities for research. The book makes a number of significant contributions to the field. First, it offers a deeper understanding of the evolution of research on BMI through an ontological map that identifies the key thematic areas in the literature. Second, a multilevel model is developed that clarifies the concept of BMI by identifying its drivers, contingencies, and outcomes. Third, the authors identify clear and specificdirections for further research and offer suggestions on research design, creating an informative road map for the future. This book adds to the debate on the effects of covenants on third-party creditors (externalities), which have recently become a focus of discussion in the contexts of bankruptcy law, corporate law and corporate governance. The general thrust of the debate is that negative effects on third-party creditors predominate because banks act in their own self-interest.After systematising the debated potential positive and negative externalities of covenants, the book empirically examines these externalities: It investigates the banks’ factual conduct and its effects on third-party creditors in Germany and the US. The study’s most significant outcome is that it disproves the assumption that banks disregard third-party creditors’ interests.These findings are then interpreted with the tools of economic analysis; particularly, with the concept of common pool resources (CPRs). Around the aggregated value of the debtor company’s asset pool (as CPR) exists an n-person prisoner’s dilemma between banks and third-party creditors: No creditor knows when and under what conditions the other creditor will appropriate funds from the debtor company’s asset pool. This coordination problem is traditionally addressed by means of bankruptcy law and collaterals. However, the incentive structure that surrounds the bilateral private governance system created by covenants and an event of default clause (a CPR private governance system) is found to also be capable of tackling this problem.Moreover, the interaction between the different regulation spheres – bankruptcy law, collateral and the CPR private governance system - has important implications for both the aforementioned discussions as well as the legal treatment of covenants and event of default clauses. Covenants alone cannot be seen as an alternative to institutional regulation; the complete CPR private governance system and its interaction with institutional regulation must also be taken into consideration. In addition, their function must first find more acceptance and respect in the legal treatment of covenants and event of default clauses: The CPR private governance system fills a gap in the regulation of the tragedy of the commons by bankruptcy law and collateral. This has particularly important implications for the German § 138 BGB, § 826 BGB and ad hoc duties to disclose insider information.
