Preventive Medicine between Obligation and Aspiration
Preventive Medicine between Obligation and Aspiration is a study of ethical questions regarding mass screening, vaccination, and health policy programmes. These interventions aim to enhance public health but may also constrain personal autonomy and cause harm, and influence our moral views. So far, these issues have hardly been subject to systematic ethical analysis. This study aims to fill this gap by providing an overview of moral problems in preventive medicine and by explicating norms for good practice. Throughout the book it is argued that some moral concerns about prevention - namely concerns about medicalization - cannot be adequately grasped in terms of strict and binding moral norms. Various moral concepts and types of norms `beyond obligation' are explored and developed in order to give practical meaning to these rather vague concerns. In this way the book contributes to applied ethics as well as to ethical theory. It is of interest to professionals in public health and preventive medicine and to scholars in applied ethics and moral philosophy.

My interest in and appreciation for program evaluation began in the early 1970's when conducting a curriculum development research project at the University of Florida's P. K. Y onge Laboratory School. This interest was sparked when it became apparent that testing the success of an education program required more skills than just statistics and research methods. After pursuing additional formal schooling, I embarked on a career featuring educational program evaluation as its central thrust--as a private consultant, later in a university health sciences center involving seven academic colleges, and then in the Cooperative Extension Services of Florida and Maryland. Adding evaluability assessment (EA) to the performance of evaluations, to program development, and to teaching about evaluation has been a significant development for me personally, and I hope to those who have been participants with me in each endeavor. This book grew out of many of these experiences and involved numerous colleagues who made significant contributions. First among these is Dr. George Mayeske, Program Evaluation Specialist, Extension Service, U. S. Department of Agriculture, Washington, D. c.

The proportion of elderly people continues to increase in the western world-nearly a quarter of the population will be over 65 years by the year 2050. Since aging is accompanied by an increase in diseases and by a deterioration in well-being, finding solutions to these social, medical and psychological problems is necessarily a major goal for society. Scientists and medical practitioners are therefore faced with the urgent task of increasing basic knowledge of the biological processes that cause aging. More resources must be put into this research in order to achieve better understanding of the cellular mechanisms that underlie the differences in life span between species and to answer the difficult questions of why some individuals age more quickly than others, and why some develop liver problems, some have heart problems, and others brain problems. The results of such a wide program of research will provide important information about the causes of many life-threatening and/ or debilitating diseases of old age; it will help find ways to prevent some of the ailments that result from aging, and it may well lead to discoveries enabling the prolongation of human life.
