Based on current research data the effects of probiotics on periodontal health and its maintenance are not clear. Scientific evidence is poor in this area. Systematic in vitro studies are first needed to learn more about the eventual interactions of probiotic species and periodontal pathogens and oral biofilms, and also about their effects on periodontal host tissue reactions. Thereafter, properly controlled, randomized, long-term clinical trials are needed using the most promising strains and also testing various methods of application.

Biomarkers have been classified by Perera and Weinstein based on the sequence of events from exposure to disease. Though biomarkers readily lend themselves to epidemiological investigations, they are also useful in the investigation of the natural history and prognosis of a disease. Biomarkers can also provide insight into disease progression, prognosis, and response to therapy. There are two major types of biomarkers: biomarkers of exposure, which are used in risk prediction, and biomarkers of disease, which are used in screening and diagnosis and monitoring of disease progression. Background & Objectives: Treatment of periodontal disease in recent years has moved toward anantibiotic/antimicrobial model of disease management. Time has come to shift theparadigm of treatment from specific bacteria elimination to altering bacterial ecology by probiotics. With the increase in the incidence of resistance to antibiotics, probiotics may be a promising area of research in periodontal therapy.Given the emergence of antibiotic resistance and the lack of non-antibiotic treatment options, A "Guided Pocket Recolonization" (GPR) approach may provide a valuable addition or alternative to the armamentarium of treatment options for periodontitis. With this objective in mind, the present study was carried out to assess and evaluate the use of a synbiotic preparation consisting of (Streptococcus faecalis, Clostridium butyricum, Bacillus mesentericus, and Lactobacillus sporogenes) as a guided pocket recolonisation (GPR) procedure in patients with chronic periodontal disease.
