Die akute Lungenentz ndung (Acute Community Pneumopathy, CAP) bei Erwachsenen stellt ein Problem f r die ffentliche Gesundheit dar und ist mit einer hohen Morbidit t behaftet. Die R ntgenaufnahme des Brustkorbs ist die einzige diagnostische R ntgenuntersuchung, die in erster Linie empfohlen wird. Eine Computertomographie (CT) ist in der Regel dann angezeigt, wenn es keine bereinstimmung zwischen Klinik und R ntgenbild gibt. Ziel unserer Studie war es, den diagnostischen Beitrag dieser Untersuchung bei Patienten, die wegen CAP ins Krankenhaus eingeliefert wurden, und ihre Auswirkungen auf das therapeutische Management zu bewerten.

Acute community-acquired pneumonia (CAP) in adults is a public health problem with a significant morbidity and mortality. Chest radiography is the only radiological diagnostic test that is recommended as a first step. Computed tomography (CT) is usually indicated in case of clinico-radiographic discordance. The aim of our study was to evaluate the diagnostic contribution of this examination in patients hospitalized for CAP and its impact on their therapeutic management.

Le tabagisme serait un facteur de risque possible du syndrome d'apn e obstructive du sommeil (SAOS) mais son influence sur la s v rit notamment l'importance de la somnolence diurne reste peu document e. Les donn es scientifiques valuant l'association du tabagisme et le syndrome d'apn e du sommeil reste limit es et controvers es. Souvent les chantillons de personnes tudi es sont trop petits pour d tecter une r elle diff rence entre fumeurs et non-fumeurs. La fum e de cigarette peut modifier les propri t s m caniques et neuronales des voies a riennes sup rieures. Le tabagisme est aussi associ une inflammation des voies respiratoires et un oed me ce qui entra ne une diminution du calibre des voies respiratoires ce qui impacte l'architecture du sommeil avec une plus grande latence l'endormissement et un d placement vers les stades l gers du sommeil induisant une somnolence duirne excessive. A travers cette monographie on vise tudier l'influence du tabagisme sur la s v rit du SAOS et son impact sur l'importance de la somnolence diurne au cours du SAOS. Le sevrage tabagique serait la pierre angulaire dans la prise en charge de cette population.

Smoking is a possible risk factor for obstructive sleep apnea syndrome (OSA), but its influence on the severity and importance of daytime sleepiness remains poorly documented. The scientific data evaluating the association of smoking and sleep apnea syndrome remains limited and controversial. Cigarette smoke can alter the mechanical and neural properties of the upper airway. Cigarette smoking is also associated with airway inflammation and edema, which leads to a decrease in airway caliber, which impacts sleep architecture with a greater latency to sleep and a shift to light sleep stages inducing excessive sleepiness. The aim of this monograph is to study the influence of smoking on the severity of OSA and its impact on the importance of daytime sleepiness during OSA. Smoking cessation would be the cornerstone in the management of this population.
