Mentre i pazienti affetti da malattia di Alzheimer (AD) hanno un rischio maggiore di sviluppare crisi epilettiche, l'attivit epilettica innesca una serie di risposte inibitorie compensatorie nei circuiti ippocampali per contrastare gli squilibri nell'attivit della rete, interferendo con le normali funzioni neuronali/sinaptiche necessarie per l'apprendimento e la memoria. Poich si sostiene che i farmaci antiepilettici (AED) modulino in modo variabile la cognizione, viene sollevata una preoccupazione riguardo agli AED pi indicati.Gabapentin e levetiracetam hanno migliorato significativamente il deterioramento cognitivo indotto dalla colchicina, lo stress ossidativo e la riduzione del livello di BDNF. Il gabapentin ha aumentato l'attivit dell'AChE, mentre il levetiracetam e il donepezil l'hanno ridotta. Nonostante l'aumento del livello di BDNF. Mentre la carbamazepina non ha migliorato significativamente il deterioramento cognitivo indotto dalla colchicina.I nuovi AED, in particolare il levetiracetam, hanno migliorato la funzione cognitiva e hanno ovviamente protetto sia l'ippocampo che la corteccia prefrontale dagli effetti deleteri della colchicina. Grazie alla sua significativa neuroprotezione, il levetiracetam potrebbe essere l'AED pi adatto a essere utilizzato nella demenza simile all'Alzheimer.
