The Perils of Patient Government

The Perils of Patient Government

In 1964 the Senate Committee on Aging reported that ""once admitted to an institution ... the veteran begins ... to show signs of social and physical degeneration,"" a phenomenon that has not escapted the attention of clinicians, social scientists, veterans, and other chronic-care patients. Assuming that social withdrawal in the institutional setting was avoidable ad that a strictly medical model of chronic care was inappropriate, Lella and his collaborators established a patient-government project designed to give thirty elderly men in a large veterans' hospital, who suffered from various degrees of social withdrawal, an opportunity to express their individuality and independence and to shape institutional decisions. The Perils of Patient Government goes well beyond a description and analysis of the projects' successful side - a general improvement in the lives of the veterans on Ward 23; it also exposes and analyzes the project's failures, portraying negotiation and conflict among change-oriented and conservative staff of varying professional identities, ideologies, and career strategies. While struggling over the idea and practice of patient self-government, nurses, and other professionals did make progress but also set severe limits on what patients could achieve for themselves. As well, Lella's study tackles the larger question of how change affects organizations and institutions. Lively and well-written, this is an enlightening work for students of gerontology and geriactics, for professionals and para-professionals, administrators, and policy-makers involved in chronic care, and for researchers probing the fields of medical sociology and institutional organization.

In the decade since the publication of the second edition of Scanning Electron Microscopy and X-Ray Microanalysis, there has been a great expansion in the capabilities of the basic scanning electron microscope (SEM) and the x-ray spectrometers. The emergence of the variab- pressure/environmental SEM has enabled the observation of samples c- taining water or other liquids or vapor and has allowed for an entirely new class of dynamic experiments, that of direct observation of che- cal reactions in situ. Critical advances in electron detector technology and computer-aided analysis have enabled structural (crystallographic) analysis of specimens at the micrometer scale through electron backscatter diffr- tion (EBSD). Low-voltage operation below 5 kV has improved x-ray spatial resolution by more than an order of magnitude and provided an effective route to minimizing sample charging. High-resolution imaging has cont- ued to develop with a more thorough understanding of how secondary el- trons are generated. The ?eld emission gun SEM, with its high brightness, advanced electron optics, which minimizes lens aberrations to yield an - fective nanometer-scale beam, and “through-the-lens” detector to enhance the measurement of primary-beam-excited secondary electrons, has made high-resolution imaging the rule rather than the exception. Methods of x-ray analysis have evolved allowing for better measurement of specimens with complex morphology: multiple thin layers of different compositions, and rough specimens and particles. Digital mapping has transformed classic x-ray area scanning, a purely qualitative technique, into fully quantitative compositional mapping.

Screaming for Change advances an understanding of punk rock by going beyond description of punk as a musical, political, social, and cultural genre of communication. Previous scholarship about punk rock has primarily dealt with those boundaries of genre. Previous scholars neglected to examine the ideology of punk across the decades and continents. That ideology, in a word, is deviance. Through Gramscian textual analysis, this book uncovers this ideology of deviance with some surprises along the way. Students and scholars of punk rock will value the book's attention to both well known and more esoteric punk artists. Punk is arguable the most studied 'subculture' to ever launch itself onto the larger social agenda as a possible counterbalance to the mainstream cultural hegemony. During the late 1970s, punk scenes sprouted up in large numbers all over the globe, and it appears that deep feelings of discontent towards the inherent alienation present in the capitalist system were the motivational seed that facilitated their growth. Unconvinced that the historical accounts have been successful in adequately describing and proficiently capturing the essence of punk, this study examines the phenomenon in slightly different terms. This study proposes that punk should be understood as a way of seeing the world, as a way of reasoning, or, essentially, as a philosophy on its own terms.
