Make-believe has great adaptive value and helps the average individual to function better in cultures saturated with puzzling contradictions. This book traces the origins of pretending (illusion-construction) and the developmental phases of this skill. Further, it analyzes how parents depend on pretending to secure conformity and self-control from their children. It unravels the ways in which make-believe is utilized to defend against death-anxiety and feelings of fragility. It examines the relationship between pretending and the classical defense mechanisms -- and particularly weighs the evidence bearing on the potential protective power of embracing religious beliefs. Finally, it defines the diverse contributions of make-believe to the construction of the self-concept, the defensive maneuvers typifying psychopathology, and the maintenance of somatic health. In short, this book pulls together a spectrum of scientific information concerning the defensive value of illusory make-believe in coping with those aspects of life -- such as death, loss, suffering, and injustice -- that are experienced as unreasonable and beyond understanding. Among the topics most closely treated here are the French mineralogical mapping project initiated by Guettard; contemporary efforts to interpret the earth historically (such as through Noah's Flood); and difficulties presented by the vocabulary often used in traditional histories of geology. Much of Rappaport's research addressed two problems prevalent within 18th-century earth science: the proper understanding of petrifactions, or fossil objects; and struggles to establish reliable knowledge of the earth's past. She also examined the chemistry of G.-F. Rouelle, which she saw as effectively an attempt at systematic comprehension of the entire mineral realm; trans-national features of scientific pursuits as illustrated in the careers of the naturalist Vallisneri and the mineralogist (and philosophe) d'Holbach; and aspects of science's promotion in France through government patronage and academic privilege.

Die vorstehenden Ausfuhrungen verfolgen an dieser Stelle vor allem den Zweck, die erhoehte Aufmerksamkeit der am Problem der Krebs- bekampfung besonders interessierten Kreise auf die in Deutschland, wohl zu Unrecht, zur Zeit nur wenig beachtete und augewandte Diathermie- behandlung boesartiger Neubildungen hinzulenken. Es wird dargelegt, dass diese Art des therapeutischen Vorgehensingeeigneten Failen mit besonderem Vorteil an Stelle des ublichen chirurgischen Verfahrens und an Stelle der Strahlenbehandlung oder in Verbindung mit diesen Metho- den angewandt werden kann. Auf die Kombination von strahlenthera- peutischen Massnahmen mit chirurgischer Diathermie wird hinsichtlich Indikationsstellung und Ergebnisse naher eingegangen, da auf die sich hieraus ergebenden therapeutischen Moeglichkeiten in der vorliegenden Literatur, soweit uns bekannt, nur andeutungsweise bisher hingewiesen wurde. Ein kurzer geschichtlicher UEberblick zeigt, dass die grundsatz- lichen Vorzuge des Diathermieverfahrens bei der Behandlung boesartiger Neubildungen schon vor 2 Jahrzehnten erkannt wurden und dass es auch in Deutschland nicht an Bestrebungen gefehlt hat, dieser Behandlungs- art Eingang zu verschaffen. Besondere Verdienste hat sich hierbei Nagelschmidt erworben. In den neuzeitlichen Krebsbekampfungszentra- len sollte neben dem Chirurgen und dem Radiologen ein mit der Dia- thermiebehandlung vertrauter Therapeut nicht mehr fehlen. Auf der anderen Seite ist weder der Wissenschaft und dem arztlichen Ansehen noch dem Kranken gedient, wenn etwa die chirurgische Diathermie als "neues Heilmittel fur bisher unheilbare Krebse", als "unblutiges Opera- tionsverfahren" oder als "Ersatz fur die Strahlentherapie" hingestellt wird.

With obesity rising at alarming rates in the US, UK, and developed countries around the world - so much that the US Department of Health has officially classified obesity as an illness and the World Health Organization has dubbed it a pandemic - weight-loss surgery is also on the rise. Traditional weight-loss programs - diet, exercise, and pharmaceuticals - are only effective for significant and sustained weight loss for about 10 percent of all people who try. The results are even more dismal for the obese aiming to lose not just a few pounds, but 50, 75, or more. And their health lies in the balance, because obesity or morbid obesity (100 pounds or more overweight by medical standards) increases by 50 to 100 percent their risk of developing heart disease, high blood pressure, diabetes, and certain cancers. Weight-loss surgery, however, is effective in bringing 80 percent of obese people to or close to average weight for their height, explains Dr. Hamilton, an Instructor at Harvard Medical School. And most sustain that weight loss for at least 10 years.Hamilton doesn't perform such surgeries, but this Tufts-educated doctor has more than general medical insight. She had the surgery herself six years ago, reducing from the size 20 she was then to the size 8 she is now. Surgical weight loss is unequivocally more effective than any other method, she says. Certainly there are complexities, risks, and some grueling decisions involved. But life as an obese person can be more risky to health and more grueling to mental health, she adds. The National Institutes of Health apparently agree, as NIH has issued recommendations for morbidly obese people to have weight-loss surgery. In the United States alone weight-loss surgeries have risen from 12,700 in 1988 to more than 78,000 in 2005. Still, fewer than one percent of the patients who fit the requirements for weight-loss surgery are ever referred to such a specialist, says Hamilton. And in blacks, where the prevalence of obesity is even higher, the referral rates are even lower, she adds. Hamilton makes clear the rewards, and the risks, of surgery that reduces stomach size or removes a piece of the intestine so calories cannot be absorbed. This book includes interviews with previously obese males and females who've had the surgery, as well as descriptions of the procedures, recovery times, costs, and insurance issues.
