Als wichtigste Indikationen fur den Fixateur externe gelten die 2 Degrees und 3 Degrees offenen Frakturen, infizierte Pseudarthrosen sowie Korrektureingriffe bei Achsenabweichungen und Langendifferenz. Der Fixateur externe wird weiter eingesetzt zur raschen, initialenRuhigstellungvon Weich- teilverletzungen und Frakturen bei Schwerstverletzten, zur Ruhigstellung geschlossener Frakturen mit schwerem Weichteiltrauma (Quetschung des Weichteilmantels, Verbrennun- gen, dermatologische Erkrankungen), bei ausgedehnten Schaft-und periartikularen Bru- chen, zur vorubergehenden gelenkuberbruckenden Ruhigstellung schwerer Weichteil-und Bandverletzungen, bei bestimmten Beckenringverletzungen und ausgewahlten Kinderfrak- turen sowie fur Arthrodesen und Osteotomien. Der Fixateur externe besitzt die einzigartige Moeglichkeit, Knochen-und Weichteile weit ausserhalb der Operations-oder Verletzungszone zu stabilisieren. Falls korrekt angewendet, gewahrt er ungehinderten Zugang zu den entscheidenden Skelett-und Weichteilstrukturen, so dass sowohl Initialversorgung als auch sekundare Wiederherstellung des Knochens und Weichteilmantels ermoeglicht wird. Durch die Verwendung des Fixateur externe bleibt das zusatzliche Gewebetrauma zur primaren Skelett- und Weichteilverletzung sehr gering. Auch das Infektrisiko ist geringer als das bei einer "inneren" Osteosynthese.

In the early 1950s, the pioneering work of Robert Danis on operative treatment of fractures was in danger of falling into oblivion. Maurice E. Muller, impressed and intrigued by his contacts with Danis, first critically applied internal fixation and immediate mobilization to some 80 patients and found the basic concept confirmed, but in need of further develop- ment with regard to technology, clinical application, and scientific analysis. In 1958 he assembled a group of friends, general and orthopedic surgeons, willing to invest time and effort in helping to create the necessary armamentarium and to form a study group for clinical trials. This group was set up in the same year under the name Arbeitsgemein- schaft fiir Osteosynthesefragen (AO), later on to be known in English-speaking countries as the Association for the Study of Internal Fixation (ASIF). The first report on operative treatment of fractures by Muller, Allgower, and Willeneg- ger, published in 1963, stressed the advantages of early open reduction and internal fixa- tion. This book, first published in German, amusingly lost an important part when trans- lated into English. At the suggestion of worried American and English partp. ers, a picture series showing the healing pattern of 188 tibial fractures operated on in Chur between December 20, 1961 and April26, 1962 was left out.

1m allgemeinen Teil dieses Manuals sollen experimentelle und wissenschaftliche Grund lagen und Prinzipien der stabilen Osteosynthese nach der AO-Methode, Funktion und Hauptindikation der verschiedenen AO-Implantate, Handhabung des Standardin strumentariums der AO sowie einige wesentliche Gesichtspunkte der Operationstechnik und der Nachbehandlung dargestellt werden. R T IL LL MI 1 Ziele, Grundlagen und Prinzipien der AO-Technik Hauptziel jeglicher Knochenbruchbehandlung ist die Wiederherstellung der vol/en Funk tion der verletzten Extremitat. Jede Fraktur stellt eine Kombination von Weichteil-und Knochenschaden dar. Sofort nach der Fraktur und im Laufe der Reparationsvorgange treten lokale Zirkulationssto rungen, entziindliche Erscheinungen und Schmerz mit reflektorischer Immobilisierung auf. Diese drei Faktoren fiihren in Verbindung mit dem Ausfall funktioneller Beanspru chung von Knochen, Gelenken und Muskeln zur sog. Frakturkrankheit, d.h. chroni scher Odembildung, Atrophie der Weichteile, Knochenabbau (Osteoporose) und Ge lenksteifen. Jede Behandlung einer Fraktur muS deshalb sowohl den Bruch an sich als auch die damit in Verbindung stehenden lokalen Reaktionen des Organismus beriicksichtigen."
