Allzuoft empfindet der Augenarzt angesichts eines neuen Patienten mit Uveitis ein GefUhl der Hilflosigkeit. Aus Unsicherheit iiber die Atiologie der Erkrankung ordnet er eine groBe Zahl von Laborunter- suchungen an (sie sind nicht immer notig), und beginnt eine Corti- costeroidtherapie. Diese oft inadaquate Versorgung ist einerseits Folge einer gewis- sen Vernachlassigung dieser Krankheitsgruppe wahrend der Ausbil- dung zum Facharzt, andererseits durch die Abhangigkeit von Lehr- biichern bedingt, die eine groBe Anzahl uveitischer Syndrome beschreiben und in alphabetischer Reihenfolge anfUhren, wobei weni- ger klinische als vie1mehr wissenschaftliche Gesichtspunkte im Vor- dergrund stehen. Ungliicklicherweise klagen die Patienten in der Praxis des Augen- arztes aber nicht iiber die Beh etsche Erkrankung, die Sarkoidose, oder das Vogt-Koyanagi-Harada-Syndrom. Sie klagen vie1mehr iiber Schmerzen, ein "rotes Auge", Lichtscheu, Verschwommensehen, "Fliegen vor den Augen" und andere Probleme. Das Spektrum der klinischen Zeichen reicht von einem reizfreien Auge mit zystoidem Makulaodem und Zellen im Glaskorper bis zu einem Auge mit knall- roter Episklera, groBen speckigen Prazipitaten auf der Hornhauthin- terflache, einem Hypopyon, und Exsudaten urn die RetinageraBe. Gelegentlich kann man auch nichtophthalmologische klinische Zei- chen, wie etwa ein entziindetes Gelenk oder Hautveranderungen zu- gleich mit der Augenerkrankung beobachten. Es miissen auch zahl- reiche andere wichtige Gesichtspunkte in Erwagung gezogen werden, die nicht unmittelbar mit diesen Symptomen zusammenhangen. Zum Beispiel kann die Erkrankung einseitig oder beidseitig auftreten und der Beginn kann beinahe unbemerkt oder sehr plotzlich verlaufen.
